Those eligible for the $80 cash back rebate is valid only for new Qwest residential customers who purchase residential service on or after [DATE]. This offer expires on [DATE] and redemptions must be postmarked by [DATE]. To qualify, your Qwest residential service must be activated before [DATE]. After submitting your request, you will receive your $80 rebate check after 10-16 weeks.

 

In order to receive the $80 rebate, your Qwest High-Speed Internet® service must be active for a minimum of 60 days. QwestDeal.com is independently operated by DigitalMojo, Inc working with Qwest Corporation to bring you this $80 cash back rebate offer. This offer is not available directly from Qwest.

 

Please review these terms and conditions of this promotional offer:

  • Purchaser must be the end-user and not a distributor, reseller, or manufacturer agency/
  • You must be a resident of the 50 United States, excluding territories.
  • You must be over 18 years of age.
  • This $80 rebate offer is available to new Qwest High-Speed Internet residential customers and former residential customers whose previous accounts were in good standing when service was terminated.
  • If you were a former Qwest residential service customer, you must not have had Qwest High-Speed Internet service during the previous 120 days.
  • To receive the $80 rebate, you must complete the original rebate form in its entirety and submit a copy of all pages of your first month's Qwest High-Speed Internet bill.
  • Photocopies of the rebate redemption form will not be accepted.
  • Your rebate request must be postmarked within 60 days from the date of your Qwest high speed internet purchase.
  • Rebates are limited to one rebate per person, per product, receipt, household, family or address.
  • The mailing address on your rebate must be the same as the address as your Qwest High Speed Internet residential service.
  • The $80 rebate value will not exceed purchase price of the Qwest high speed internet service.
  • Franchise fees, taxes, and other fees may apply, with the actual amount depending on location and services ordered. Certain restrictions apply.
  • Rebate requests from P.O. Boxes or groups will not be accepted.
  • Your mailing address must be valid. Rebate forms containing undeliverable mailing address will be denied.
  • DigitalMojo, Inc is not responsible for misdirected, late, lost, damaged or postage-due mail.
  • You cannot transfer your rebate rights to another person.
  • This $80 rebate offer is void where taxed, restricted or prohibited by law.

If these terms and conditions are not met, your rebate will not be issued.

 

Please make and keep copies of all materials submitted. Your original documents become the property of DigitalMojo, Inc and will not be returned. Submissions that are invalid submissions will not be returned.

 

Fraudulent Submissions 

If we feel necessary, DigitalMojo, Inc reserves the right to request additional information from you regarding claims. These claims are subject to review under US Postal regulations. Submitting fraudulent rebate forms could result in federal prosecution under mail fraud statues (Title 18, USC Sections 1341 & 1342). Proof of mailing does not constitute proof of delivery.

 

Rebate Fulfillment 

Your rebate checks will be remitted in US dollars. Your rebate check will be mailed 10-16 weeks after you have completed the qualifying activity. If you have not received your check within 16 weeks of submitting rebate, please send an email noting the date you submitted you rebate for to sales@qwestdeal.com.

 

In the event of an address, change, it is your responsibility to provide mail forwarding. If you have not cashed or deposited your check after 60 days of issue, any obligation of DigitalMojo, Inc is null and void. We will not issue refund checks.

 

Our Rights and Responsibilities 

DigitalMojo, Inc is not responsible for lost, stolen or fraudulent use of checks. The rebate form alone has no cash value. If your Qwest high speed internet service has been canceled before the 60-day minimum needed to qualify for the rebate and the rebate has been redeemed, DigitalMojo, Inc reserves the right to recover the full dollar amount of rebate.

 

Qwest Corporation and DigitalMojo, Inc reserves the right to change the Terms and Conditions or end the offer at any time without notice. DigitalMojo, Inc serves strictly as a rebate intermediary for purposes of this offer and disclaims any liability associated with the $80 cash back rebate offer.
